The message below is from DHS. Please make sure to complete and return you reevaluations on time. If you are a registered provider we suggest taking the training now so you don't have to scramble to take it when you receive the form.
Message from DHS
Attention Child Care Providers for DHS:
All DHS child care providers are required to complete the listing re-evaluation process every two years. Direct Pay Unit will send your two year reevaluation to you on your due date. You will need to complete the form and return to DPU by the due date stated in your cover letter in order for you to remain in approved status and continue to receive future billing forms. Providers will not receive child care payment while in a failed status.
The requirements to be a child care provider for DHS have recently changed. Information regarding the provider requirements can be found on the Provider Information Sheet which is attached to the Provider Listing form. Reminder: Registered Family Providers will need to complete the Basic Child Care Health and Safety training or have completed Oregon Kids Healthy and Safe (OKHS) training, before you submit your re-evaluation to Direct Pay Unit (DPU).
If you have completed the Oregon Kids Healthy and Safe (OKHS) class, you will need to attach a copy of your certificate to the enclosed re-evaluation to show that you have completed the class.
